Location: Avera at Home Mitchell Worker Type: Regular Work Shift: Primarily days, with night, weekend, and holiday on-call involvement Pay Range: $31.00 - $46.25 based on experience Position Perks: • Potential eligibility for a $10,000 sign-on bonus • Possible relocation bonus • Work hours: M-F, 8 AM - 4:30 PM, with occasional weekend & on-call Student Loan Repayment: This position may qualify for Avera's Student Loan Repayment Program, offering monthly payments of $167 (up to $10,000) to help reduce your loan principal faster. Position Overview: As a Visit Nurse, you will utilize the nursing process to deliver exceptional care to Avera@Home patients, adhering to the ANA Standards of Clinical Nursing Practice. You will be accountable for the nursing care provided in patients' homes or facilities, ensuring quality outcomes and safety. Your Responsibilities: • Collect thorough patient health data. • Analyze assessment data to determine nursing diagnoses/problems. • Identify individualized expected outcomes/goals for each patient. • Develop comprehensive care plans with prescribed interventions. • Implement care interventions as per the plan. • Evaluate patient progress toward achieving outcomes. • Continuously assess the quality and effectiveness of your nursing practice. • Collaborate with patients, families, and healthcare team members in patient care. • Incorporate research and evidence-based practices as appropriate. • Consider safety, effectiveness, and cost in patient care planning and delivery. Essential Qualifications: • Valid driver's license required. • Active RN license in the state of practice is necessary. Preferred Qualifications: • Minimum of two years of related experience, or an equivalent combination of education and experience.
