What Makes This Role Stand Out: • Consistent Weekend Hours: Fit work around your life with weekend-only scheduling • Excellent Pay: $200 per Start of Care visit + mileage • Focused Role: Specialize in new patient admissions and care plan initiation • Community-Based: Get out of the office and into the homes of clients across the metro • Professional Independence: Use your clinical judgment to help patients begin their care journey Primary Responsibilities: • Perform weekend Start of Care (SOC) admissions with complete OASIS documentation • Develop and initiate care plans in collaboration with the interdisciplinary team • Evaluate patient needs and coordinate necessary services • Educate patients and caregivers on disease management and home care best practices Required Qualifications: • Valid Minnesota RN license and CPR certification • Proven experience in home health and OASIS documentation • Reliable transportation, driver's license, and active insurance • Strong communication and clinical assessment skills Applicable MOC Codes: We welcome all active duty, reservists, and veterans.
