As a Hospice Visiting Nurse (RN), you will:
• Support patients, families, and loved ones through their end-of-life journey.
• Perform comprehensive assessments to establish goals for symptom management and end-of-life care.
• Participate in an interdisciplinary team that holistically addresses the needs of patients.
• Develop and maintain therapeutic relationships through effective communication with patients and families.
• Utilize cutting-edge technology to document the skilled services you provide.
Qualifications:
• Active Registered Nurse license in Connecticut.
• One year of RN experience is required; Homecare or Hospice experience is preferred.
• Valid driver's license and reliable transportation are necessary.
• Proficiency in computer skills is required.
