The role involves managing 3-4 admissions on Friday, Saturday, and Sunday, with Monday mornings allocated for paperwork and attending a monthly nurse meeting. Your Responsibilities as a Home Health Admissions RN: • Conduct thorough initial assessments to determine patient eligibility and care requirements. • Initiate the Plan of Care based on physician orders, clinical standards, and regulatory guidelines. • Accurately document all patient assessments, clinical findings, and admission records in a timely manner. • Act as a liaison between referral sources, patients, families, and the interdisciplinary team to facilitate a supportive onboarding process. • Ensure compliance with all applicable federal, state, and agency policies. • Maintain professional nursing standards while providing compassionate, hands-on patient care. Qualifications - What You'll Bring: • Active RN license in the state (or eligibility to obtain). • At least 1 year of high acuity nursing experience. • A compassionate, flexible, and patient-centered approach to care. • Ability to provide care in various home and community settings. • Strong organizational, communication, and clinical decision-making skills. • Valid driver's license, automobile insurance, and reliable transportation. • Current CPR certification. Preferred Experience (Not Required): • 1+ year in home health, hospice, ICU, geriatrics, Med-Surg, ER, PACU, telemetry, cardiac, or similar clinical settings. • Background as an Admissions RN, in wound care, float pool, or travel nursing.
