
Weekend Registered Nurse (Friday to Sunday) in Port Barre, LA
Job Description
As a Weekend Registered Nurse, you will take full nursing responsibility for implementing the Plan of Care for each hospice patient. You will continuously assess and enhance both personal and professional performance to improve productivity and quality of care.
Key Responsibilities:
• Conduct initial nursing evaluations to determine eligibility for hospice services during visits within forty-eight (48) hours of referral.
• Identify and reassess the physical, psychosocial, and environmental needs of patients and their families every fifteen (15) days or as required.
• Develop and coordinate the plan of care while maintaining precise and relevant clinical documentation on the patient’s condition.
• Document issues, set appropriate goals, initiate interventions, and track patient/family responses to hospice care.
• Work collaboratively with patients, families, attending physicians, and other members of the interdisciplinary team (IDG) to deliver comprehensive care on a daily basis.

Required Qualifications:
• Current and unrestricted RN licensure.
• Valid Driver's License, vehicle insurance, and access to dependable transportation.
• Current CPR Certification.
• At least 1 year of clinical experience.
• Able to function in diverse home settings regardless of client background.
State Specific Requirements:
• 2+ years of full-time experience as an RN is required. However, two years of experience as a licensed practical nurse in hospice care may be substituted.
Preferred Qualifications:
• Able to work independently.
• Strong communication, writing, and organizational skills.
The salary for this position ranges from $58,800 to $105,000 annually, depending on experience and qualifications. Along with competitive pay, we offer a comprehensive benefits package, including 401k contributions and recognition programs, all subject to eligibility requirements.
