Key Responsibilities Collaborate to provide management and delivery of patient care plans Support patients in Mount Lebanon PA and surrounding areas Meet critical deadlines for charting Self-manage time and schedule Demonstrate experience with tablets mobile phones and EMR software Schedule : Weekend F / T flexible hours (3-12s is normal but open to accommodating other scheduled Travel Local travel required Qualifications Required • Associate degree or higher • 1 years of experience in ANY of the following : Electronic Medical Record (EMR) • Other EMR • TruChart EHR • Valid Registered Nurse (RN) in Pennsylvania (PA) • Hospice (2 years) • Do you have a valid Drivers License Reliable Mode of Transportation and Auto Insurance (all 3) Are you okay with the required travel for this position Preferred • Please provide your RN License number.
