We are seeking a Full-time Weekend Registered Nurse who is flexible but is available Saturday and Sunday. As a Home Health Weekend RN, you will be a crucial field employee responsible for:
• Conducting skilled nursing visits and coordinating overall client care
• Serving as the primary point of contact for all disciplines involved in patient care
• Overseeing visit frequency throughout the care episode
• Consulting with physicians as needed and providing detailed patient care updates
• Accurately collecting OASIS data and maintaining current medication profiles
• Ensuring timely communication of lab values to physicians
• Participating in weekly case conferences and monthly case manager meetings

Qualifications for the Weekend Registered Nurse position include:
• Current Registered Nurse (RN) license in your state of employment
• Minimum of one year of clinical experience preferred
• Solid knowledge and skills in current nursing practices
• Valid state driver’s license and automobile liability insurance
