This role requires availability on Saturdays and Sundays with flexible scheduling. The Home Health Weekend RN will: • Conduct skilled nursing visits and coordinate comprehensive client care • Serve as the primary contact for all disciplines involved in patient care • Manage the frequency of visits for each episode of care • Consult with physicians and office staff as required regarding patient care • Ensure accurate OASIS data collection and maintain current medication profiles • Report lab values to physicians in a timely manner • Participate in weekly case conferences and monthly case manager meetings Qualifications To apply for this position, candidates must meet the following qualifications: • Current Registered Nurse (RN) license in the state of employment • Minimum of one year of clinical experience is preferred • Strong knowledge and skills in current nursing practices • Valid state driver's license and automobile liability insurance Additional Information Enhabit Home Health & Hospice is an equal opportunity employer committed to promoting diversity and encouraging a collaborative and respectful work environment.
