
Wellness Director - Registered Nurse in Beech Grove, IN
Job Description
Royal Faith Homecare Agency is seeking a compassionate and qualified Wellness Registered Nurse (RN) to support individuals with intellectual and developmental disabilities (I/DD) receiving services in their homes and communities. This role supports BDDS-funded services and focuses on health oversight, wellness coordination, and regulatory compliance while promoting independence and quality of life.
Essential Job Duties
• Serve as a member of the interdisciplinary team in accordance with BDDS service standards
• Conduct nursing assessments and ongoing health monitoring for individuals served
• Develop, implement, and maintain individualized Wellness Coordination Plans under RN supervision, as applicable
• Monitor and supplement nursing care needs as directed by the supervising RN
• Administer medications and train staff on proper medication administration, documentation, and MAR compliance
• Ensure health, safety, and well-being of individuals while promoting dignity and self-determination
• Assist individuals with learning appropriate self-care and health maintenance skills
• Provide staff training and on-site education related to health needs and nursing supports
• Maintain accurate clinical documentation in compliance with Indiana BDDS, Medicaid, and agency policies
• Report incidents, health changes, and concerns per BDDS reporting requirements
• Collaborate with case managers, physicians, guardians, and other providers as needed
Minimum Qualifications
• Graduate of an accredited nursing program
• Active Indiana Registered Nurse (RN) license in good standing
• Ability to pass a criminal background check per BDDS requirements
• Valid driver’s license and reliable transportation
• Be able to go on licensure for the state
• Current auto insurance meeting agency standards
• Ability and willingness to travel within the assigned service area
• Strong clinical judgment, documentation, and communication skills
• Commitment to person-centered care
• Experience working with individuals with intellectual or developmental disabilities preferred
