Job Description:
The Registered Nurse provides comprehensive psychiatric nursing services to individuals with severe and persistent mental illness in an Adult Outpatient setting. Care is delivered through ambulatory services at the 2 Wall Street clinic and in community-based settings. Responsibilities include psychiatric assessment, medication administration and monitoring, care coordination, patient and family education, crisis intervention, and collaboration with an interdisciplinary treatment team to support recovery, stability, and continuity of care.
Requirements
Key Responsibilities:
These services consist of assessment and education regarding medications, administration of injectable medications, and administration of AIMS exams. Coordination of services with case managers and prescribers as needed. Fundamental computer experience to process e-mail and client access is required. Medication reconciliation, being a liaison between patients and their treatment team to ensure coordinated care, participating in team meetings, and providing feedback and observations to the clinical team.
Patient Care Support: This may include injections, medication monitoring, and education, nursing assessments including AIMS exams and Clozaril monitoring.
Requirements
Qualifications: A New Hampshire RN license is required, as well as certification as a psychiatric and mental health nurse from the ANCC when eligible. Current CPR and BLS certifications required as well
Experience: Must have at least one year of work experience in the field of psychiatric/mental health services. ANCC preferred
